The formula
An insulin concentration states units per milliliter -- U-100 means 100 units/mL, U-40 means 40 units/mL. Dividing the unit amount by that concentration gives the volume containing it:

What does U-100 mean?
U-100 insulin contains 100 units of insulin in every milliliter -- the standard concentration for most human insulin sold in the US, stated directly on FDA-approved product labeling (FDA-approved product labeling, Humulin® R (insulin human injection, USP), "100 UNITS PER ML (U-100)").
What does U-40 mean, and where is it used?
U-40 insulin contains 40 units per milliliter -- less concentrated than U-100. In the US it's mainly seen in FDA-approved veterinary insulin such as Vetsulin for dogs and cats (FDA Freedom of Information Summary, NADA 141-236 (Vetsulin®, porcine insulin zinc suspension), "40 international units (IU) insulin/mL"; "U-40 insulin syringe"); it's also still used for some human insulin products in other countries.
Why does the same number of units give a different volume on U-40 vs. U-100?
Because "units" measure insulin activity, not volume -- a U-40 vial holds the same 40 units in more liquid than a U-100 vial holds in the same volume. 25 units is 0.250 mL of U-100 insulin, but 0.625 mL of U-40 insulin -- two and a half times as much liquid for the identical unit amount.
Can I draw insulin units with a syringe marked in mL?
Insulin syringes are marked directly in units matched to a specific concentration (a U-100 syringe for U-100 insulin, a U-40 syringe for U-40 insulin) specifically so you never have to convert units to mL by hand at the point of injection. Using the wrong syringe for the concentration in the vial is a well-documented dosing-error risk -- always match the syringe to the vial.
