Why mg to ml needs a concentration

Milligrams measure a mass of active ingredient; milliliters measure a volume of liquid. A liquid medicine dissolves a certain mass of active ingredient into each unit of volume -- its concentration, printed on the label, usually as "X mg per Y mL." Dividing the mg amount by the mg-per-mL concentration gives the volume that contains it:

mL = mg ÷ (mg per mL)

Frequently Asked Questions

How many ml is 500 mg?

It depends on the concentration -- 500 mg only converts to a volume once you know how many mg are in each mL of the specific liquid. At 32 mg/mL (a 160 mg per 5 mL label), 500 mg is about 15.6 mL; at a different concentration, the answer changes. There is no fixed mg-to-ml answer without a concentration.

What does "160 mg per 5 mL" on a label mean?

It states the concentration directly: every 5 mL of the liquid contains 160 mg of the active ingredient, i.e. 32 mg per mL. This is the standardized concentration FDA guidance describes for children's/infants' liquid acetaminophen (FDA guidance, "Over-the-Counter Pediatric Oral Liquid Drug Products Containing Acetaminophen" (December 2011 industry transition to the single standardized 160 mg/5 mL concentration)) -- always read the concentration on the actual bottle in front of you, since not every product uses this number.

Is this the same as a dosing calculator?

No. This is pure arithmetic on a concentration you provide (mg ÷ mg-per-mL = mL) -- it does not know a dose, an age, a weight, or a maximum daily amount. Dosing decisions belong to the product's Drug Facts label, a pharmacist, or a prescriber, not this page.

What's the difference between the two calculation methods here?

"From a label" uses a concentration already expressed in mg per mL (the format almost every liquid medicine label uses). "From a substance's density" is for a pure substance where you know its density in g/mL rather than a diluted concentration -- a different calculation, included for completeness, not the common case for medicine.
